Working Geometric LOS

1. Does your facility use GLOS/GMLOS for working DRG assignment?
2. If so, who is responsible for assigning GLOS/GMLOS?
3. When is GLOS/GMLOS assigned? (on admisison, within 48hrs, etc)

Our case management department has requested the CDI dept provide
working DRGs to them because they do not assign DRGs. We are wondering how other facilities handle this.

    Angie,
    We actually just started working with our case management department on this very issue. We had an outside consultant who developed a communication program for CDI/CASE MANGEMENT. Our CDI department assigns the DRG within 24-48 hours of arrival. Once the CDI team documents the DRG a GMLOS is auto calculated. The care management department is then responsible for looking at the LOS.

    We do use the GLOS to assign a working DRG. The CDS is responsible for assigning this to the pt chart.

    Are you meaning a anticipated discharge date? Our MD's are now obligated to note anticipated discharge date from the time of admission. In order to do this, they have found it helpful to have access to the GMLOS of the associated working DRG. We do assign working DRG's and note that in our EMR in a format that is viewable by case management and the MD's so that they can use the DRG associated GMLOS to estimate the patients expected LOS (anticipated discharge date).

    I am not sure if that is what you are referring to. If not, sorry!

    Hello,

    We put the working DRG which then calculates the GLOS into the Meditech abstracting module. IT then was able to "grab" that data and place it on the census sheet that the CM and charge nurses print daily.

    So you code in your CDI system then go back and place it in the Meditech abstract section, I have wanted to have this happen at our facility, put I really would not like to work in two places. We have to keep the use of the 3 M CDIS system to keep track of the data.

    Has anyone started to use CAC, and if so does the product you use allow for this data to interface.
    We use 3M and Meditech.

    We put together a Laser Arc Report which has DRG/LOS for each patient by unit for CM. The CM's can pull this report every morning. It reflects the current DRG and LOS.

    It pulls the data from what we enter into our encoder (Nuance Clintegrity) and feeds directly into this report.

    McKesson (STAR) is our abstracting software.


    Of course...when I worked in UM...we all knew how to code and assign our own DRG/LOS...just sayin'...

    Case Management sends me a list of patient’s with > than 3 day LOS (Medicare and DRG Payors) twice a week and depending on our workload -I either send it back with the GLOS, or attend a thirty to forty minute meeting that afternoon with them and the Physician advisor and provide it then.
