Escalation Policy Help!!

Does anyone have a sample escalation policy for clinical documentation or quality/risk/cores that they would be willing to share? We recently have acquired a CMO who will be dealing with many issues that before now have been unattended to. Since we haven't had this luxury before, we would like to have an escalation policy in place for all to follow.
