transbronchial lung biopsies

We have gone through 3 coding supervisors since my time here and they all have said different things re what documentation is required to capture code for TBLBx. Our trainers say dr. must document tlbx AND path report must show alveolar, parenchymal, or lung tissue. if not successful, or if bronchial mucosa is only obtained, evenn though attempt was made, we do not have a reimbursable procedure.

now i am hearing just making an attempt is enough.

does any have a coding clinic on this? also if they say endobronchial is that reimbursable?

There is a coding clinic to refer to. Coding Clinic second quarter 2009 pg 16, 17. We had a chart with a transbronchial biopsy that was requested by our RAC. They used the above coding clinic as their reference. What if the path report isn't back yet? I believe that the documentation must include transbronchial biopsy and the use of fluoroscopy because it is not biopsy by direct visualization.
