Medicare discharge notice

This isn't a CDI question, but I'm involved in the process at our facility and I'm wondering if anyone knows how their facility handles the medicare discharge notice. Our facility is trying to streamline this process and we are having difficulty coming up with a good process.

  • Well I was in case management at our facility for years and the case managers have this signed the day before planned dc or the day of dc they take them back to the rooms and have the patient or family sign. If the patient can't sign or no family available, they have them mailed to the family after the fact or try to call and do a verbal; our admissions have them sign one on admit and if it is over I believe the time frame till dc the above is what is done. Hope this makes sense and helps?

    Our Admitting department is responsible for having the initial Important Message from Medicare (Form CMS-R-193) signed by the patient or family. We have four Office Assistants in Case Management who are responsible for IMM's after the initial one has been signed. The Office Assistants tried working with the CM's to find out discharge dates, but that did not work too well - too many were missed. Now their process is to give each patient an IMM every two days - that is the only way that they found to not miss any. Extra work, yes; but they are 100% compliant.
