OB Chart reviews

Hi
Does anyone do OB chart reviews? If so, what are some of the main documentation issues you see in this area? What are the main complications, etc?

  • We do not do concurrent reviews on OB charts. We do post discharge queries for issues such as acute blood loss anemia and insufficient prenatal care. They are the most common, but I only do about 6 a month. That is a very small percentage of our admissions. Our population is mostly uninsured and medicaid.

    We don't do OB, only GYN surgeries.

    I do OB reviews @ our hospital and I review UHC, SELF-PAY, PASSPORT ADV, MEDICAID and the occasional MEDICARE. My #'s average about 12-20/month and it's hardly worth the trouble. Any patient that is acutely ill is transferred to a larger hospital that can provide a higher level of intensive care for the baby. Most of the questions I ask are for acute blood loss anemia. The OB/GYN's are good about documenting no PNC on their own. I also cover ICU and the Medical Oncology floor and that is much more satisfying.
