CDI mixed with CM

Does anyone know of a great article regarding the fact that CDI and CM should not be melded together? One will certainly fail.

    My research project for my Master's degree was concerning case management models and their impact on 30 day readmissions for AMI. I don't have an article that indicates that they shouldn't be combined roles, but I have an article that describes that combining roles requires very high functioning in both UR and CDI. That model does not work well unless the staff are very experienced in both roles. Otherwise, the program will not be as strong as a program that UR and CDI and separate and distinct.
