Establishing criteria for Respiratory Failure

I have a question for my CDI associates out there. In a week and a half I am going to meet with our organizations' Pulmonologists. I have already sent them a rough draft for acute Respiratory Failure criteria with SOI and intensity of service using Milliman criteria and the criteria from the CDI Pocket Guide book for 2014. I want to establish our organization's criteria for Acute Respiratory Failure as our ACDIS associates suggested at the 2014 conference and I have seen it discussed in this blog. Do any physicians or other CDI professional have any suggestions or other resource to offer? Points for persuasion during our meeting? I have a lot of critical care experience and CM experience doing UR so I have some depth for this task. Help!

Subject: [cdi_talk] Doc asking for info on reimbursement

While posting a query the doc wants me to include the potential reimbursement with it. Do you not provide it & tell him its inappropriate for me to do so? Consult my director? other suggestions?

  • I've attached a copy of a monthly documentation tip that I produce on respiratory status. Additionally, Paul Evans had previously shared the second attachment via CDI Talk on respiratory also -- a most excellent resource!

  • Thanks, Don

My material may be a bit dated as I wrote this a 'long time ago'....just a heads-up on this complicated topic.

I actually really found the discussion Dr. Pinson provided @ ACDIS regarding the P/F ratio to be very useful. The tool can be very useful when no RA ABG is available or of the pulse ox readings seem unclear. Suggest this as resource, too.

  • I agree with Paul, in that the P/F ratio can be a powerful tool to use in cases of acute respiratory failure. The information on the P/F ratio in the 2014 CDI Pocket Guide is very informative.
