Opioid dependence

I have a physician group that is struggling with calling this diagnosis out. Does anyone have any information/articles/thoughts about this diagnosis?? Thanks for your help

  • When I query I write-

    Pt home med oxycontin, 30 mg, q 8 hrs
    oxycodone, 10 mg, q 4 hr, prn break thorugh
    Hx- chronic back pain, 3 prior back surgeries

    Please clarify if there is any diagnosis associated with the above clinical evidence and medication usage:

    Opioid dependence, continuous/daily use
    Opioid dependence, intermittent/not daily use
    No evidence of opioid dependence
    Other
    Unable to determine.

    I put the word daily in there as a clarification, and I explain to them:These are frequently the patients that stay longer because their pain make all of their issues harder to treat. I say we are just capturing the acuity and the resources needed to care for these patients. It shows the extra time they require and often the longer length of stays.

  • Here I am again 6 years later and I am having the same issue with one of our phsycians and the use of opioid dependence for patients who are on chronic pain medications. We were directed by our consulting company to query for this when patients are on chronic pain medications. Are others querying for this? What definition do you use to support "dependence". Thanks for your assistance :)
