CDI and Coders-DRG Discrepancies/Issues

Our CDI team usually e-mails the coders to discuss DRG discrepancies/issues. However, we would like to use a standardized form/tool to track our work.
What process do you have in place at your hospital? Do you use a standardized form/tool that is used by both the coders and CDI?
