Katy I think your facility has the correct approach. Why would we ever follow-up with providers to answer a query if the documentation is already in the record. The one exception is if there is conflicting documentation.
At the end of the day clear, accurate & ideally consistent documentation has to be our top goal not high query compliance rates. If we produce high CMI, CC/MCC capture rates, & high multiple CC/MCC rates with accurate documentation who really cares about the other stuff.
