It sounds potentially very leading and thus non-compliant. I wouldn’t ever tell a provider what to write. The only thing I have done is consolidate multiple queries, when appropriate to do so, into one email for the provider to address. When doing verbal I will address all queries at one time but never tell them what to write.
Sounds like a questionable practice at best.
