Pneumonia organisms

Anybody out there? Phoning a friend for the million $! I am only 7 mos. in this field and am a "lone ranger" without you guys.

Hospitalist documented Pneumonia, bacterial organism, suspect gram negative and gram positive. Treated with Vancomycin 750 mg IV Tues, Thurs, Sat IV, Zosyn 2.25 mg IV q 6 hrs (ESRD on HD).

Pt fits in category of suspecting Gm negative Pneumonia in absence of positive culture- 1. dialysis patient 2. Inability to produce adequate sputum sample 3. Prescribed extended spectrum Penicillin (Zosyn).

Can code DRG 177 be used when suspected negative and positive organisms? Or do you have to stick with DRG 693?

    693??? DO YOU mean 195? I would use 177. If you feel like you are on the fence, query. They are treating as such is gram neg. MANY times the cx comes back negative!

    You have suspected (probable) and adequate treatment (Vanc and Zosyn) for DRG 177. Yes, I would take this to 177.
