Query for Cardiogenic shock, hemorrhagic shock

Can anyone provide me with some assistance regarding writing a query for cardiogenic and hemorrhagic shock?
No matter how I word my query my MD's will only document CHF, anemia, etc
I provide the patients clinical picture - ask for a diagnosis related to the clinical picture, ask for the professional opinion - I end up right back where I started.

    I often will see the documentation include "hypotension" but not shock. I present the clinical information but then query for the etiology or more specific diagnosis for the hypotension. I also include that IV fluids, vasopressors, and/or blood was administered.

    This usually helps them realize I need more than just GI bleed with hypotension or MI with hypotension especially when the patient is in ICU.

  • I look at shock as a collapse of the peripheral circulation, as manifested by critically low blood pressure readings (hypotension). A query would be indicated for shock when the patient's blood pressure is unable to be maintained by volume replacement (IV fluids, blood) and the blood pressure now requires support with vasopressors.
