How to code an "evolving" condition

The physician is documenting Clostridium difficile colitis with “evolving” toxic megacolon. Do I code toxic megacolon as if it exists as the definition of "evolving" is to develop gradually or does the "impending" or "threatened" coding guideline apply?

Appreciate any thoughts and/or suggestions.
