Good afternoon,
I am a CDI, subscribe to CDI Talk, and have a question regarding Hyperammonemia. I had a patient with Cirrhosis of the liver . The patient has documented Hepatic Encephalopathy and MD continued to document elevated Ammonia level. Patient received lactulose and Ammonia level improved after several days.
Is it appropriate to query for Hyperammonemia when hepatic encephalopathy has been documented ? Is the hyperammonemia considered intrinsic to the hepatic encephalopathy ? My email address is sca4@aol.com Thank You in advance for your response , Cyndy Anderson ,RN, CDI,

  • In my view, one should not code the elevated level of ammonia in a pt w/ documented hepatic encephalopathy (Chronic Hep C??) as elevated level is a typical feature of the code assigned for the diagnosis.
