ICD 10 questions about addendums

Today I had a physician mention that per an online education he took, he was provided information that he needed to have his diagnoses and supporting documentation all on chart prior to him writing DC order. He mentioned that addendum's could not be carried out in ICD 10. Anyone have any info to share ?
