We are seeing coding denials from Humana for Acute Renal Failure. The rational they are giving is that the diagnosis of Acute Kidney Failure will only be considered after achieving an adequate status of hydration and after excluding urinary obstruction. Also criteria for Acute Kidney Failure will not apply if patient is on diuretic.
On admission the BUN was 78, Cr 4.6, Na 124 This did improve with IV fluid.
Day of D/C Cr 0.8, BUN 12

So I wondering if others are seeing this type of denial and if so what are you doing about it?
