wound care documentation
We have a new wound care NP starting and she will be documenting in the provider section of the medical record.
As she is an NP, we will be able to use her documentation for coding purposes.
She is documenting the presence of skin tears/shearing injuries.
Can anyone weigh in on the appropriate code for a skin tear?
When I group skin tear, I'm getting to a laceration code which doesn't seem right. I found one article that said it should get coded to a superficial wound.
Obviously, it's better we capture what is occurring with the patients-I just want to make sure we are assigning the most appropriate codes.
