Acute Resp Failure due to CHF

What treatment qualifies for diagnosis of Acute Resp Failure with CHF exacerbation? We often see IV Lasix of course, as well as nebulizers and low dose of O2. But technically does not meet the treatment to validate the diagnosis of respiratory failure. The patient does meet diagnostic testing and clinical evidence. Have you had denials because of lack of treatment?
