Acute and Chronic

Okay guys, I know this may seem like a stupid question to some but I'm just curious now with ICD-10-CM and needing to be more specific and all.

If a patient comes in with acute cholecystitis with cholelithiasis.
The pathology report states only chronic cholecystitis with cholelithiasis,

Why can't it be coded with both an acute and chronic cholecystitis. Why can't the pathology report we used to provide the additional further specificity.

Does the final diagnosis really need to say acute and chronic cholecystitis with cholelithiasis? I realize you can't take a diagnosis from a pathology report without the physician confirming and documenting it but why can't we use the additional chronic to provide more specificity.

I thought with ICD-10 we could use specificity in path reports.
