Chronic lung disease/BPD

I apologize in advance if this has already been discussed-I did a search of the message board and these terms did not yield any results.
I am interested in finding out how others are addressing the issue of clarifying for BPD when terms like "chronic lung disease of infancy" and "chronic lung disease of prematurity" are documented in the medical record. Are you clarifying specifically for BPD by providing clinical indicators re: FIO2 and length of oxygen use per NICHHD criteria? Are you even able to clarify if the FIO2 and length of oxygen use is not available in previous records or not known? This is assuming of course that the infant/child was not discharged home from the NICU on oxygen (those cases are much more clear cut!)
