  • I am not involved in outpatient CDI but my facility has hired staff for Ambulatory CDI. I think it would be wonderful to have a resource those involved in CDI in the Outpatient area.
    I will be meeting with the new CDI educator for this unit and provide her with as much information as possible but I am sure there are some differences between inpatient and outpatient.
    Our goal is to ensure we are as consistent as possible when we educate our physicians.

  • I agree w/ sentiments expressed. The coding and charging rules and conventions for outpatient are very different from those for acute, facility-based (DRG) programs. I used to have some expertise in the outpatient arena, but it has been a long time since I worked in the arena. I'd love for my site to use an outpatient program that could compliment the inpatient program.

    I would welcome an outpatient CDI book! If possible, could you have it ready in a few weeks, lol. I have been working on developing a best practice outpatient model and gathering information from programs across the country is an hefty undertaking to say the least. Since there are so many pathways CDI can impact in the ambulatory setting, I am wondering how you would address the majority in the book and still provide a good resource for program implementation/improvement, metrics, staff training/education, facility buy-in and on-going monitoring. All the different pathways is also a major issue facing software vendors and developers in coming to market with an outpatient platform. I did have to limit my options and am targeting the physician practice setting with a focus on risk adjustment/HCC methodology. I would really like to see how others have implemented a program like this and how they are collaborating with traditional inpatient CDI programs to impact RAF scores.

  • I think this is a topic we definitely need a book on. Something like the CDI Handbook but focused on OP. We have considered an OP program many times and tend to end up back where we currently are because it's confusing and overwhelming to consider implementing an OP program.
    I think we need better guidance on a few things:
    1. What are we calling OP: Obs, ED, OP surg, hospital clinics, etc? all of the above? One of the most difficult parts of considering an OP program is that OP encompasses a HUGE area. What are we really talking about?
    2. What is the outcome benefit of OP CDI (lets get real, most hospitals aren’t going to hire additional staff unless there is ROI: Financial or in quality metrics)
    3. Who is best positioned to do this work? Interestingly enough, IP CDI has moved to being in many facilities (including mine, unfortunately) relegated to RN's. When I have considered an OP program the first thing I realize is that I do not think it would be easy at all to extend current staff into this role. I honestly don’t think they have the knowledge. This doesn’t mean they couldn’t learn. But true OP CDI seems to be much more closely linked to billing and insurance than IP. I tend to think that an experienced coder with good OP knowledge of billing and insurance issues may be best to move into this role. I would be curious what facilities with programs in place are going.
    4. How do we best identify areas of risk? In the past I have been told to look at OP denials. Still the case? Or are their standard areas that seem to be high risk.
    5. Info on workflow, structure, FTE's, metrics, etc...
