severe sepsis
Help!
All of a sudden the physicians (ED and Hospitalists) are documenting Severe Sepsis and basing it on the lactic acid and maybe leukocytosis or tachycardia or tachypnea.....
i.e. Severe sepsis based on Lactic acid of 3.8, bolus given, pt. also with tachycardia; 93, WBC 12.4. CXR left lower lobe infiltrate. Pneumonia. Admit: cardiac monitoring, IVF's repeat lactic acid.
Literally there is not any other clinical documentation to support (severe) sepsis....this has been occurring often over the past 3-4weeks.
Anyone else seeing a spike? Is it due to the fact that the "sepsis campaign" has changed?
How are you handling this?
This is being documented throughout the chart and the discharge summary.
Input? Thoughts?....I know time for education, for me and them.
