Pt admitted with severe chef ph R HEART failure and less talked about Left ventricular systolic CHF. RD document severe protein calorie malnutrition- 13 % wt loss 6 m with less 50% intake greater than 5 days, muscle wasting and lsevere loss of fat stores. Query issued for severe PCM... Reply " cardiac cache is -not sure how that codes?"

And weight loss due to RV failure...any advice?
Codes to heart failure unspecified with cachexia.

Does that preclude pt from having SEVERE. PCM?
