PHYSICIAN QUERIES

Just recently started the CDI program at my facility. Is there any harm in having too many types of queries? I know the goal is to get the physicians to document in the progress notes, but since this is all new to them it is difficult. Our queries are a permanent part of the record. A small handful of physicians that I have educated are doing well, but there are many in which are not. My co worker wants to have a query for every thing ie: resp failure, surg debridement, renal failure, eetc. if this is the case then we could have a chart with 3-4 queries. i would love to know anyone's thoughts.

    When I have more than one query, I use a general query sheet. I limit the number of queries on the sheet as well. I have had much better response by limiting my query sheet to 1 or 2 questions. I am careful when I have multiple questions-asking myself if it is going to affect the DRG, affect SOI/ROM, or an audit risk. If it doesn't fall into one of those categories, then I usually don't ask. If there are that many issues on the chart, it might be in your best interest to have a one on one with the physician.

    I have sent multiple queries to providers depending on the situation. If I can I consolidate them, but if they go written, I will send a different sheet for each query, that way I can log the response to each issue. Sometimes I'll get one addressed and the other ignored and this gives me an accurate response rate.

    We have around 20-something standard queries. We can customize them for our specific needs. If I have more than one I combine them to a single sheet as best I can.

    At the "Dear Dr." I draw their attention to the fact there is more than one query. I then state in parentheses how many (2) and number them on the sheet.

    When one is answered I check it off, initial and date it and leave it for them to see that I am still waiting for an answer for the other one.

    Sometimes I get a better response when they see that I am reviewing and making note of the documentation.

    Our queries are not a permanent part odf the chart.


  • We have many different query forms. The question is how many queries do you ask at one time. You do not want to bombard them with too many questions - they won't answer any of them. You need to prioritize your queries as to their importance to that specific chart. I try not to leave more than 2, once in a while 3, at a time. It depends on what I am asking.
