    If you are bipolar I, mixed = 296.6x

    Bipolar disorder is 296.80, but there is a 5th digit category for 296.6X and Atypical is 296.7

    Doing a direct code in the encoder:

    296.89 = bipolar disorder

    Bipolar II Disorder 286.89 CC Therefore, we have discussed here our clarification opportunities with this in order to capture the CC/severity. If just bipolar disorder is documented, we will write a clarification to further specify the bipolar disorder as type I or type II. We only ask these clarifications of hospitalists, internists, or intensivists. We would not address this with surgeons.

  • Karen, if it is type I, you would need further specificity as to whether the behavior is manic, depressive, etc. Just Bipolar I disorder without specification goes to 296.7, which is not a CC.

    Thanks. I can see where there was some confusion in my original response because there was a type for the code I gave for Bipolar Disorder Type II. It should have read 296.89.
    In formulating our strategies, we were looking at the medical/surgical patients with just bipolar disorder documented. So, simply having it clarified as type I or type II, considering in most instances (except psych admits) the condition would be stabilized on meds, if it was Bipolar type 1 NOS (296.7) it would not be a CC, but if type II (296.89) it would be a CC. For the most part, without a psych consult, the other specificities would not known.
