NEW SEPSIS GUIDELINE QUERY!!

Attention all fellow CDI cohorts...do any of you have a query form for SEPSIS based on the new SEPSIS guidelines and regarding the SOFA/qSOFA score? I am from a CAH and we query using paper forms. Our facility will be adopting the new SEPSIS guidelines. ANY and ALL help will be much appreciated. THANK YOU!!
