Initial Review

Hi Everyone,

I would like to ask the group what is the length of time from admission to your initial review.  I am researching best practices due to so little information provided on initial admission on some accounts and the impending change of other accounts to observation status.   Thank you for your feedback.

    24 to 36 hours allows time for initial provider documentation and diagnostic results to be available for the initial review.

  • We average somewhere between 36-48hrs (closer to 48). We still have a hybrid record so @ 48hrs we have an h&p and one progress note to review.

    Has anyone found that moving from hybrid record to fully electronic allows you to review earlier or review more records?

  • Hello, since our records are all EMR - we review on day 2 - I agree that day 1 reviews are 'fruitless' for clinical content and ability to determine PDX.
  • we used to say 24-48 but I have started suggesting more closer to the 48hr mark. This is because in general the CDI's are not comfortable querying in the first day because they want to give the provider a chance to work up the patient and document appropriately prior to query placement. To me, there is no point in performing a review if you will not query on any issues at that time.

    True Katy...Seems like 48hrs is the magic hour.
