placing dietary consults

In past, it had been advised not to put in dietary consults directly- CDIs. If there appeared to be a needed evaluation, either call dietician or have nursing put in consult was the advice.

It had been stated that an auditor could challenge the treatment of the RD was caused by the CDIs ordering of the consult and might be considered a conflict of interest and only the active treatment team should be putting in any orders.

How do others handle this? Do you see any conflicts in CDI entering a Nutrition consult?

  • We cannot enter any consults as CDSs. We review BMI z-scores or weight/length z-scores (pediatric facility) and diet tech notes. If documentation by providers indicate that child is underweight or appears malnourished and there are risk factors (ie documented history of weight loss, etc) and patient does not qualify for RD assessment based on diet tech's algorithm, we will notify the RD by pager that patient did not qualify for RD assessment but has risk factors and may benefit from an evaluation. Our RDs do not require MD order for evaluation.
