Would you code excisional debridement with this documentation?
Hi everyone!
Could you please tell me if you would code excisional debridement based on this documentation? And why or why not?
“An incision and drainage and debridement of the bullous lesion/hematoma was performed at bedside. An iris scissors was used to puncture the lesion and excision of the undermining skin was performed"

I would. I think the two sentences are describing the same procedure. The second sentence is providing more detail about how the I&D and debridement were done and what exact structure was excised. I will be interested to see what others think!

I would. We have a debridement documented as well as supporting documentation stating that skin (level) was excised (type) with scissors (instrument).

So far our coders do not think it meets criteria for 'excisional debridement". CDI and Coding have a meeting later on and we would like to discuss this there. Thanks for the input.
