metabolic coder discussion

Hi Fellow CDI, I have a child that was admitted with a history of a metabolic disorder. She had been febrile for a couple of days prior to admission with a max of 40.2c. Within 24 hrs of admission  she was diagnosed with and ear infection. She was also at risk of a metabolic stroke due to her fever. She was treated with torodol, RTC Tylenol, a cooling blanket, and ice packs, D10 IV fluids. She was also diagnosed on day 3 with RSV.

CDI's PDX was the metabolic disorder

Coder PDX was ear infection

What got her admitted, was the fever in combo with the metabolic condition. This child was really sick, and what kept her admitted was the fact that she had a metabolic disorder with fever. Child was sent home on Amox. for the ear infection.

Similar to a sickle cell pt with a fever.

I am asking what would you code as the PDX?

  • Would a child necessarily need to be admitted for an ear infection?  I can say from personal experience I never had been admitted.
