Glasgow coma scores

Has anyone developed a policy / procedure / guidelines regarding when to code Glasgow coma scores.  We want to have some guidelines for our coders directing them when they should code the scores. Should the lowest score be coded? Our records could have 3 Glasgow scores in ED plus scores each shift or more.  How do you pick which scores to report?

    we don't have a 'policy' however the guidelines advise "at minimum, report the initial score documented on presentation at your facility". So I guide our team that the initial GCS should be coded and then if they have a significant decompensation, I advise that we code the lowest GCS as well.
