Medicare Cost Outliers

Hi,

Does anyone track/review or otherwise look at Medicare Cost Outliers? If you do, how do you track them? What do you do to impact this? and can anyone provide some information on this? I have to put together a work plan to routinely address these cost outliers and have no idea where to begin.
