Incidental Pregnancy

We have a GI provider who occasionally treats pregnant patients. He does no like to document the pregnancy as being incidental because he feels that he cannot determine whether the pregnancy affects the condition he's treating and vice versa. Does anyone have articles or good sources that address when to document a pregnancy as incidental? The guidelines really put this back on the provider and I'd like to have good information for him. Thank you for any help you can provide.
