Asthma exacerbation and copd not exacerbated.

Pt had copd exacerbation and resp insufficiency per admit inpt order and H&P.  Per progress note asthma exac with acute resp insufficiency...continue copd pathway with IV steroids, bronchodilation, supplemental oxygen.  D/C summary acute asthma exac with acute resp insufficiency.  HPI of d/c summary past med hx of pertinent for asthma and copd.  Query placed if pt had both asthma ex and copd exac...query answered, "unable to determine but clinically asthma exacerbation only".

I reviewed the Coding HB 19 Diseases of the resp system/asthma...but can't find a perfect coding clinic.  

What would be the pdx asthma exacerbation with copd, unspec secondary?
or copd, unspec as pdx with asthma exacerbation secondary?

    Based on the information you provided, I would use Asthma as PDx.  The query response supports asthma exacerbation as the acute condition treated.
