ACUTE EXACERBATION CHF WITH NO TREATMENT

Physician documenting  acute on chronic diastolic CHF with no IV diuretic treatment and no change in home meds.   Could someone please advise me what they do in these situations as far as querying.   When no treatment to support the acute phase of the CHF.

    This type of situation is covered in great detail in the Clinical Documentation Article recently released by ACDIS and available on this site.   The last few pages have sample queries that I believe you may find applicable.


    I'd issue a query for validation.

    Go to 'resources' page and either use the search feature, or select position papers, resources, etc.  I can't paste a copy here, but it is pretty intuitive.  Search for 'clinical vaildation'
