RE:Process for physician documentation of postdischarge query responses
There are some hospitals that do add queries as deficiencies, and it can help.
For most, the real question behind this: what are the politics? would we be allowed to do this?
Alas, for us the stars do not yet align.
As far as the original question -- for the first day or so after discharge, the CDS will call/contact the physician seeking final clarification. All CDS query responses are via PN, etc. (not the query form). After that point, it is up to the coders to submit the question to the physician if still warranted. Coding queries and responses are part of the legal medical record.
All of our queries (CDS & Coder) are electronically submitted via in-basket (go into separate folders, with different titles and different 'boiler plate' format of the introductory text).
