Is there any guidance on coding new onset as Acute Heart Failure?

    New onset combined Heart failure is documented. A Coder has requested a query for Acute or Chronic. Any guidance?

    Even if/when acuity of a condition is clinically obvious, the Coding Guidelines require the condition be explicitly stated as 'acute' - the coder is applying the guidelines properly.
