Morbid Obesity Query

I am wondering what format you all use to query for morbid obesity.  In a scenario that the physician has not documented anything about the weight or BMI, yet I see that per nursing documentation, the BMI is >45,etc.

  • check the nursing notes for the patient's weight and height, then query the physician using this information. I think a yes/no query format will be appropriate if you have the information.

  • If there is no Medical diagnosis in the chart and you have the BMI as your clinical indicator (may have more if they require assistance OOB, two person assist, etc...) you’d need to query the provider using a multiple choice format. 

    The BMI can be taken from calculations from nursing, Dietician, etc.. 

    Yes/no formats should be reserved for those situations when you are asking a Provider to agree with a diagnosis documented by a consulting provider, non treating provider such as Radiologist or Pathologist, etc...
