Pressure Ulcer with Flap
Coding of Pressure Ulcers when there is a flap- Example: pt has a stage 4 pressure ulcer and has a flap done. Pt discharged to SNF, then readmitted to the hospital a few days later. Documentation states stage 4 pressure ulcer with muscle flap, “healing’. How do you code that? (do we need a query for unstageable??)
I would think if the flap was completely healed, we would code nothing as the pressure ulcer is gone??
