query responses

we are trying to streamline our query response options and curious what others are doing and/or best practice.  what we've found is that an 'unable to determine" response on a BMI of 58 if that is obesity is counted as a "response" but not an agree.  I think this hyper inflates our response rate as there was no additional documentation added to the record and we know that truly means "I don't agree with what you asked".  we are thinking the options should be - agree, disagree, or no response. 

I would love to know what your organization is doing.
