Query Retention Policy
How long do you wait before submitting the first query request ? (after the 1st PN, 2nd PN, 48 hrs). What is the query retention policy at your organization?

There are some publications I have read that suggest CDI be involved from the time the patient is seen in the ER, so that HCC's can be caught as well. This is especially important in helping the physician to formulate a treatment plan. e.g A patient comes in that is septic but also has a history of CHF, the patient may not receive the recommended fluid bolus based on Sepsis protocol due to that history.
I traditionally wait until I see a H&P, that includes the "active" problem list that the patient is being treated.
