Patient safety concerns by CDI staff

When a CDI RN finds concerning information in medical records (ie. policies not being followed in regards to staff failing to monitor patient(s) per physician orders, not intervening on findings - including critical labs - as ordered) how are your hospitals asking you to handle these findings? Or how do you handle these in organizations you have, or do, work at?

  • I would report these findings through your occurrence system for risk management to follow up on.
  • At a minimum we would complete an incident report.  Depending on the situation, we may notify other departments
