Outpatient coding/billing of services

Hi all. We have recently been going around and discussing about billing of services. If a patient as an annual wellness visit and the provider also sees the patient and does a preventative visit but the patient or the provider brings up a problem and that meets an E/M level, can you and do you bill for all three services??
