Sepsis due to unknown source of infection
We have a widespread issue with providers documenting "sepsis due to unknown source of infection". It is regardless of the sepsis criteria used; CORE, SOFA, etc. Our Quality Department has launched a campaign to assist with poor CORE measure outcomes so if the providers see minimal criteria they document sepsis even if the criteria can be explained by other conditions (elevated Cr in the presence of ESRD as an example). Queries have not helped when they have been asked for a source or suspected source of infection. My question is if any of you are seeing this same issue and if so, what are you doing to clarify the situation? Suggestions, comments, further questions, all are more than welcome.
