work flow

Hello, I have not contributed before but read regularly. I have been doing CDI for almost 2yrs and was wondering how others daily work flow went? Are you on the floors all day, do you have an office or work space? Where do you keep references and paperwork if you have it?

    I have a desk in the Case Management office. I use a laptop so most of
    my references are in there. I usually go to the floor from 7-9, so I am
    available for the docs, and I usually review the electronic data at that
    time. I go back at 1030 and look at charts until I am done.

    I have other job responsibilities such as helping with quality data
    collection and I do the IV infusion/injection charges on the observation
    patients

    We have a desk in the Med Records which is located right across from physicians lounge and a great place to catch docs. We are part paper and part electronic until Oct when we will be all electronic. In the morning we try to locate doctors we need to sign queries, review admit list for CDI cases, reviewed d/c CDI coding summaries, etc. Late AM up on floors until 3p reviewing charts. This is a typical day. There are some days where we never make it to floors due to making bulletin boards, newsletters, listening to audio conferences, meetings, etc. We are part of Case Mgt but have no Case Management or Quality responsibilities.
  • My cubicle is located in the HIM department. I spend about half to 3/4
    of my day on the floors reviewing charts and the rest of my day in the
    office doing paperwork, etc.

    I am on the floors 5-6 hours a day. I have a cart which carries my laptop and books. I have a cubicle located in HIM with the coders (my department is Quality).

    The rest of the day is data entry or the like.

    We are located in HIM.

    In the mornings I come in, run reports and then spend the rest of my day on the floors. I work primarily from one unit in the docs dictation room. I drop down and review my other floors by hand and key it in when I get back.

    We have laptops and computer bags. I am able to print from any printer on the nursing stations.

    We are hybrid- paper on the floor - except for cold-fed data- and all EMR after discharge.

    I glean admissions from the computer, identify CDI opprtunities and generate queries. I can print the query and file it on the chart.

    Mondays are usually admissions, admissions admissions. Tuesday I have a Team mtg with CM/UM/SWS./Charge nurse. Its helpfulfor identifying. Patients who are going to NH and Rehabs so we get our part of the transfer DRG. I then work on continued stays till lunch. Afterwards I glean admissions/generate queries..

    The reat of the week is rinse and repeat.

    Fridays I work from the office - admissions and then reports. (Mismatches and entering queries.)

    We have a small workspace where we run reports in the AM and do inital data input - what we can glean from the EMR. We are hybrid, and have to be on the floor to read orders and progress notes, place our questions to the docs, and look for answers.

    We are probably in the "office" for about 2 hrs in the AM (depending on # of new admits), and about 30 mins at end of day. The rest of the time is on the floor, using a laptop on a "COW".
