At Dayton Children's we haven't solely went to pSOFA, we continue to use SIRS + infection. Our CDI team looks for an altered mental status or "ill appearing" "decreased activity" for further indicators to send queries. Sepsis is definitely a tough diagnosis to know when to query!!
