carotid artery disease

Hi all, question in regards to coding carotid artery disease. If the provider doesn't mention stenosis or occlusion, it codes out as I77.9 which is HCC 108. But if the provider mentioned stenosis or occlusion, it does not code out to an HCC. This seems odd to me as one would think stenosis or occlusion would make the patient "riskier". Does anyone have any thoughts about this?
