medication without diagnosis

Just wondering if any of you query for why medication and if so how are you doing it?   I have some for instance that are on medications for depression, anxiety, hypercholesterolemia  and the medication is continued while they are inpatient but no diagnosis listed.   I have queried but the only thing I have is the medication.    Just wanted some other thoughts.

    I query for "clinical significance" when they are giving meds/continuing home meds without a diagnosis
