Uncertain diagnosis terminology

Please help us settle a small debate :)

Would you consider the following documentation regarding chronic stable angina as an uncertain diagnosis?

He does seem to have some chronic mild stable angina. This had occurs only with heavier exertion. He does also noted some dyspnea on exertion. He had 2+ edema today, and we will at least monitor his symptoms with an increase in his Lasix to 80 mg in the morning and 40 in the afternoon. He does have an updated echocardiogram ordered by the cardiologist at his last visit

  • I see it saying there is chronic mild stable angina. I do not take this as being uncertain.

    Just my opinion!

  • Thank you for your input. It might be prudent to add that the discussion is around the presentation of the diagnosis.

A more direct question would be: Does the verbiage "does seem to" translate to an uncertain diagnosis?
